Your responsibilities will include

Completing timely, strengths-basedCare Act assessments Supporting discharge planning from acute hospital and community settings Working closely with multidisciplinary teams to coordinate safe discharge arrangements CompletingMental Capacity Assessmentsand contributing to Best Interest decisions where required Arranging care packages, reablement and community-based support Promoting independence and helping to prevent avoidable readmissions Liaising with individuals, families, advocates and external professionals throughout the discharge process To be considered, you will need: A recognisedSocial Work qualification CurrentSocial Work England registration Experience inHospital Discharge, Discharge to Assess, Adult Social Care or Adult Community Services Strong knowledge of theCare Act,Mental Capacity Actand strengths-based practice Confidence managing complex cases in a busy multidisciplinary setting Experience using Mosaic, or another case management system, would be beneficial If youre looking for a meaningful role where you can make a genuine difference to adults and families at a crucial point in their care journey, wed love to hear from you.
