Complex Care Support Worker

Job Description We are looking for a warm, energetic and dedicated Complex Care Support Worker to support a young lady with complex needs who thrives on positive relationships, meaningful activities and a nurturing environment.

This role is ideal for someone who is naturally caring, enjoys having fun, and is passionate about helping young people achieve their fullest potential. You will play a key role in promoting independence, emotional wellbeing and social development, while ensuring safety, dignity and high-quality personalised care.

Working closely with families and a multidisciplinary team, you will help create opportunities that enrich daily life and support the young person to flourish. ResponsibilitiesDelivering personalised care that meets physical, emotional and social needs in line with an individualised care planSupporting daily routines including personal care, mobility, meals, communication and structured activitiesEnabling fun, confidence-building experiences at home and in the community, including outings, hobbies and social opportunitiesDriving to appointments or activities when required to support participation and independenceRecording clear, accurate notes on progress, wellbeing and any changes in presentationWorking collaboratively with families, healthcare professionals and colleagues to maintain consistent, high-quality supportMaintaining a safe, nurturing environment through adherence to safeguarding and health and safety standardsEngaging in ongoing training and development to ensure best practice in complex careSkillsExperience supporting young people with disabilities or complex needsExcellent communication skills, including clear spoken English, to build trust with the young person and their familyAbility to follow detailed care plans and adapt support to changing needsConfidence using IT systems for documentation and reportingA full driving licence is desirable but not essentialA compassionate, patient and fun-loving approach, with the ability to create positive, engaging interactionsStrong organisational skills and the ability to manage varied responsibilitiesFlexibility and resilience, with a commitment to supporting the young persons development and social outcomesDesirablePolish speakingExperience in day services with young adultsThis role offers an enriching opportunity for individuals passionate about making a positive impact through social outcomes.
