Transforming Support Coulsdon is looking for Support Workers to join the team at Chipstead Court, providing person centred support to adults with learning disabilities and complex needs. This is a practical, people focused role where you will build trusted relationships, support everyday living and help each person achieve what matters to them.

Transforming Support provides person centred supported living and outreach services for adults with learning disabilities, autism and complex needs across the UK.The RoleAs a Support Worker, you will provide consistent, person centred support that promotes dignity, independence, choice and involvement in the local community.

Your responsibilities will include

Supporting individuals with everyday living and maintaining their independenceHelping people make choices about their daily routines, activities and lifestyleSupporting access to the local community, appointments, education, employment and social activitiesProviding support with health needs and medication in line with agreed proceduresHelping individuals manage their tenancy, finances and household responsibilities where requiredBuilding positive relationships with the people you support, their families and other professionalsEncouraging independence, confidence and positive risk takingMaintaining accurate support records and completing required documentationFollowing safeguarding, health and safety and risk management proceduresWorking closely with colleagues to provide consistent, high quality supportYou will be part of a supportive team focused on delivering safe, respectful and reliable support while helping each individual retain as much control over their own life as possible.

About YouYou will have at least 6 months' previous care or support experience and a genuine commitment to person centred support. You will be reliable, respectful and confident supporting adults with a range of individual and complex needs.

Being a driver would be advantageous, although it is not essential. You will ideally bring: At least 6 months' previous care or support experienceA genuine commitment to promoting independence, dignity and choiceA reliable and responsible approach to workGood verbal and written communication skillsThe ability to work effectively as part of a teamConfidence supporting people with a range of individual and complex needsThe ability to maintain accurate records and follow agreed support plansFlexibility to work the required two week rolling shift patternA calm, respectful and person centred approachGood awareness of safeguarding, confidentiality and health and safetyA full UK driving licence would be advantageousThe right to work in the UKPrevious experience in supported living, learning disabilities or complex needs would be particularly useful.
