Field Supervisor

Overview

In this role, you will support care consultations, reviews and client care plans while guiding Care Professionals. You’ll maintain accurate records in our care systems and foster strong relationships with clients, families and professionals. You will mentor staff, support the Client Experience team and participate in on-call duties to ensure high-quality, compliant home care. The role offers a meaningful opportunity to improve independent living and well-being for seniors within a collaborative, mission-driven environment.

Responsibilities

• Support care consultations, service reviews and client support plans
• Assist with staff supervision and client QA activities
• Create, update and audit client care plans; support digital care planning
• Maintain accurate client and Care Professional records in Home Instead software and People Planner
• Conduct client and Care Professional introductions
• Build and maintain relationships with clients, families and other professionals
• Perform client reviews and take necessary actions
• Support and mentor Care Professionals
• Provide support to the Client Experience team
• Maintain regular contact with Clients and Care Professionals
• Participate in on-call duties as required
• Ensure compliance with Equality, Diversity and Equal Opportunities policy

Key requirements

• Level 3 NVQ in Health and Social Care or equivalent
• Experience in the care sector delivering personal care
• Experience with MAR sheets and medication in a supervisory role
• Passion for delivering high-quality care and promoting independent living
• Knowledge of legislation and regulations in Health and Social Care
• Good communication skills and ability to build rapport quickly
• Confident using care management technology and training Care Professionals
• Understanding of confidentiality within current legislation
• Proficiency with IT systems; familiarity with Microsoft Office or Google Suite; comfortable with virtual platforms
• Full driving license and means of transport if required within the territory
